I figured this would be a nice companion thread to the relaxing one. Yes, there are times when I want to watch something utterly terrifying, and when that happens this is the list I tend to go to:
1. THE EXORCIST (Have had this film in my collection for years, and I keep telling myself I'm gonna watch it every Halloween, but I chicken out and downgrade to THE ENTITY) 2. THE ENTITY (This movie scared me so much, I had to watch the Men At Work concert that was on right after it just to come down). 3. CABIN FEVER (This movie kicked my ass in the theater, so much so I left the theater. Now I own it. Go figure). 4. HORROR EXPRESS (Eye trauma disturbs me, and there is a lot of on display here)

The Exorcist: I don't really have a problem when watching it but I almost always have intense nightmares after.
Paranormal Activity: I'll probably get some flack for this, but this movie caused me to lose some sleep after watching it. Mainly due to the fact that a lot of odd occurrences used to (and still do) go down in the old house I grew up in. Also, I occasionally go through sleep paralysis (also known as Old Hag Syndrome) where you wake up prematurely and find that your body can't move. It's also accompanied by balls out terrifying hallucinations of acid like proportions.
Poltergeist II: The Other Side: Most of the movie I'm fine with, but the character of Henry Kane freaks me the hell out.
The Strangers: Like Paranormal Activity, it's not so much based on the quality of the movie, but more on the subject of the movie relating to my life. I live in the middle of bum fucked Egypt, and at the time was staying solo in a house that was semi-isolated away from other houses. I'd say if I watched it now, it wouldn't nearly have the same effect, but the first time I watched it, it kept me pretty paranoid for a week or so.

I would agree with the Exorcist. The Death Masks are great. They used Subliminal Seduction to fuck with people's minds. I have a couple of books that detail all of the techniques that were used in the film. They really went deep with evrything in that film all the way down to the Wallpaper they used in some scenes. The use of these techniques adds some weight to the stories of people freaking out or vomiting in the theater. I hate when people say that it isnt scary, maybe they are not picking up all of the hidden triggers. I enjoy the movie very much but it sticks in my mind for a few days. It doesn't bother me, it pretty much has become part of me. I try to embrace the fear. Movies dont scare me as much as some of the images in them. The scenes with the actual freaks in The Sentinal and the deformed sister in Pet Semetary. Paranormal Activity uses one of the tricks that was used in the Exorcist. When the sound of the Bass hum is elevated in the movie, the viewer has to listen a little closer or turn up or tune in thier senses to hear the dialogue and then A DOOR SLAMS and it startles the senses. This is a mild form of hypnosis. The grainy look of Paranomal Activity adds to this sensory adjustment. I am not a fan of the movie but I can still recognize the methods used to SCARE people. To a lesser extent the style (or lack of) in the Blair Witch with the shaky cam reportedly made people sick in the theater.

THE EXORCIST is a flick that tends to leave one "fucked up" for days afterwards.

The Sixth Sense - Still has the ability to give me nightmares. Everytime that kid says, "Come on, I'll show you where my dad hides his guns." and turns around to reveal the gunshot wound in the back of his head I freeze. I'm getting chills just writing about it.
The Blair Witch Project - Some of the most believable scenes of terror ever recorded. The ending, to this day, gets me everytime.
A Blade in the Dark - The movie itself isn't that terrifying. I mean, it's good for some tension, but that bathroom murder is still hard for me to watch.
Wolf Creek - Not only one of the greatest villains ever committed to film, but that spine severing scene is just brutal.
The Exorcism of Emily Rose - I'll admit the courtroom drama drags this one down a bit, but the actual possession scenes? The Exorcist ain't got shit on this film. The scene where she tries to reach for the cross, alone, did more for me than The Exorcist ever did...and I love me some Linda Blair.

The Sixth Sense scares the shit out of me. After seeing it in the theater I couldn't sleep. I was terrified! I buried myself in my blankets and wouldn't move afraid that that kid with the back of his head blown off was going to walk by or that girl was going to come out from underneath my bed,

Twister - Fucked me up as a kid.
Jaws - Still won't swim in open water by myself, fresh or salt.

Paranormal Activity scared me too. I have it on DVD, but I haven't watched it since it was originally in the theatres. I will watch it again one of these days.
